Arthritis is a degenerative condition of the knee causing pain, swelling and restricting movement of the
joint in affected patients. Osteoarthritis is one of the most debilitating form of arthritis and is
characterized by the deterioration of the knee cartilage accompanied by changes in the subchondral
bone and tissue of the knee.
There are three different compartments in the knee : medial, lateral and patello-femoral. In almost all
the patients, knee arthritis starts as a single compartment arthritis and gradually progresses to involve
other compartments. Diagnosis at an early stage and proactive treatment in patients with mono-
compartment arthritis can not only relieve the pain associated with this condition but can also slows
down the progression of arthritis to all the compartments.
When total knee replacement came into trend, it was considered as the gold standard treatment for
knee arthritis patient and it still is, but only in multi compartmental knee arthritis.
In today’s era, with rapid technological advancements, newer treatment options are available to tackle
arthritis early, depending upon which stage of arthritis a patient has.
These treatment options can be in the form of visscosupplementation knee injections, stem cell
transplantation, platelet rich plasma injections, bone osteotomies, knee arthroscopy, partial knee
arthroplasty to name a few.
We put all such minor procedures under a common term called knee preservative procedures. This
term is used because our goal is to preserve one’s own knee rather than replacing it with artificial
material (implant ).
Total knee replacement should be reserved for patient who have exhausted all conservative and minor
surgical treatment modalities.
All knee replacement implants have a definitive lifespan, which depends more upon the health of the
bone rather than the quality of implant . Considering total knee replacement surgery as the only
solution for all knee arthritis is not the right concept, rather treatment protocol should be decided based on thorough clinical examination, xrays, ct/mri scans of the knee joint and most importantly symptoms of patient, his/her activity level and expectations .
We adopt a multidisciplinary approach for knee arthritis patient, involving orthopaedic surgeon,
rehabilitation team and pain specialists who comprehensively assess every patient, examine, discuss and
then only offer the treatment option which we feel is the best for that particular patient, which could be knee preservative surgery or knee replacement.
In general, the results of the joint preservative surgery are very dependent upon the patient working
with our rehabilitation team following the treatment. A properly guided rehabilitation program is
essential to achieve one’s maximal outcomes after knee preservation treatment.
